What to Bring

At TCSI, we understand the challenges and concerns families have when seeking residential treatment for your teen. We strive to make the admissions process as easy as possible. What to Bring

To help streamline the admitting process, please provide the following information before and upon arrival for admission at The Center. 

Before Arrival

  • Birth Certificate
  • Social Security Card
  • Immunization records signed by a licensed physician.
  • School records (Checkout sheet, report card -if applicable-, education testing, any special education records.)
  • Legal records if applicable.
  • Medical examination signed and dated by a licensed physician, within 30 days before or after admission. Children being transferred from an agency where they have had a medical examination within the past year are exempt. Record of examination must be supplied.
  • Psychological or psychiatric evaluation report (dated) within 6 months prior to admission.

At Arrival

  • Insurance cards (medical and prescription [private insurance or Medicaid card])
  • Current prescription medications (minimum 30 day supply)
  • Medical examination signed and dated by a licensed physician, within 30 days before or after admission. Children being transferred from an agency where they have had a medical examination within the past year are exempt. Record of Examination must be supplied.
  • Authorization for treatment and consent for treatment
  • Dental examination signed and dated by a licensed dentist within one year before admission or arrangements must be made for an appointment after admission. A copy of the dental examination report and findings must be in the child's record within six months of admission.

What Not to Bring

  • Food of any kind
  • Aerosol products
  • CDs, DVDs, video games or MP3 players
  • Valuable items
  • Products with alcohol and harsh chemicals
